Referring a Patient for Rehabilitation Services

Physical Medicine and Rehabilitation

Outpatient Physical and Occupational Therapy

The Rehabilitation Medicine Department provides outpatient occupational and physical therapy services to patients referred by a licensed physician. Prescriptions for therapy must be specific and signed, and the diagnosis of the patient must require the skill of a licensed therapist.

Insurance companies may also require a referral for therapy by the primary care provider. Medicare requires that the referring physician sign and update the therapy plan of care every 90 days.

Appropriate candidates for inpatient rehabilitation patients must:

  • Be 18 years of age or older.
  • Be medically stable.
  • Require physical and occupational therapy, and speech therapy, if needed.
  • Be able to participate in intensive therapy for three hours per day, six days a week.
  • Have the potential to improve mobility and self care.
  • Require daily physician and rehabilitation nursing care.

Appropriate candidates for rehabilitation will be admitted to the unit upon financial clearance, medical readiness and bed availability.
